COVID Questionnaire

When you arrive, you will be asked to fill out this COVID Questionnaire. If you would like to print this at home, fill it out, and bring it with you, you may do so.

COVID Health and Safety Procedures

We continue to have several precautionary measures for your safety and the safety of our staff and students:

  • Clients are not required to wear masks during the session, but they are required to enter the building and in all common areas (waiting room, hallways, and bathrooms).
  • Students will completely sanitize the room between clients using a CDC approved disinfectant. In addition to our staff routinely cleaning all surfaces throughout each day.
  • Classroom and Hands-on room are disinfected daily using CDC approved products. Students will through clean hands prior to and following your visit.
  • Each guest is required to sanitize or wash hands upon arrival.
  • Guest accompanying those scheduled for services are not permitted.
  • Prepayment for services is available if clients prefer contactless checkout online or by phone.
  • We have closed our waiting room for seating. There is signage outside instructing clients to call when they arrive, and we are asking clients to wait in their car until their appointment time and we will let them know when the student is ready for them.
  • We are also staggering appointment times to minimize the number of clients in the building at any one time.
  • We are asking all clients to sign a Consent to Treat Waiver before their session.
  • We will be doing temperature checks for all students, staff, and clients. This will be done with a touch-less thermometer and takes only a few seconds. Any staff or clients with 100.4 fever or higher will be unable to enter the building.

We have recently installed a state-of-the-art infrared Air Filtration System.
